Rectal bleeding refers to the passage of blood from the anus or rectum, which may present as bright red blood on toilet paper, blood coating or mixed within the stool, or blood in the toilet bowl after a bowel movement.
While rectal bleeding is commonly caused by benign anorectal conditions such as hemorrhoids, it may also reflect more serious pathology involving the rectum or colon, including polyps, inflammatory disease, or colorectal cancer.
Even small amounts of bleeding warrant evaluation, particularly if the bleeding persists, recurs, or occurs with associated symptoms such as abdominal pain, weight loss, anemia, or changes in bowel habits.
Rectal bleeding may originate from the anus rectum or colon and can be caused by hemorrhoids fissures inflammation polyps or malignancy.
What Is Rectal Bleeding?
Rectal bleeding occurs when blood originates from the lower gastrointestinal (GI) tract, which includes the colon, rectum, and anus. The bleeding may vary in color and appearance depending on the source.
Common presentations include:
Bright red blood on toilet paper
Blood coating the stool
Blood dripping into the toilet
Dark red or maroon-colored stool
Blood mixed within stool
Bright red blood typically suggests bleeding from the rectum or anus, while darker blood may originate from higher portions of the colon.
Common Symptoms Associated With Rectal Bleeding
Rectal bleeding may occur alone or together with other symptoms. Patients frequently report:
Blood after bowel movements
Blood streaks on stool
Blood on toilet paper
Anal discomfort or pain
Itching around the anus
Mucus discharge
Changes in bowel habits
In some cases, rectal bleeding may occur without pain, particularly when caused by internal hemorrhoids.
What Does Rectal Bleeding Look Like?
The appearance of rectal bleeding can provide clues about the underlying cause.
Bright Red Blood
Often seen with:
Hemorrhoids
Anal fissures
Rectal inflammation
Dark Red or Maroon Blood
May suggest bleeding from:
Colon diverticula
Polyps
Inflammatory bowel disease
Blood Mixed With Stool
Can indicate conditions affecting the colon such as:
Colon polyps
Colorectal cancer
Colitis
Common Causes of Rectal Bleeding
Several conditions may cause rectal bleeding. The most frequent causes include:
Hemorrhoids
Hemorrhoids are swollen veins in the anus or rectum and are the most common cause of bright red rectal bleeding. Bleeding often occurs during or after bowel movements.
Anal Fissures
A small tear in the lining of the anus can cause painful bleeding, usually with bowel movements.
Diverticular Disease
Small pouches in the colon wall (diverticula) can bleed suddenly and sometimes heavily.
Colon Polyps
Polyps are abnormal growths in the colon that may bleed and can sometimes develop into cancer if left untreated.
Inflammatory Bowel Disease
Conditions such as ulcerative colitis and Crohnβs disease can cause inflammation and bleeding of the colon lining.
Colorectal Cancer
Although less common, rectal bleeding can be an early symptom of colorectal cancer, particularly in patients over age 45.
When Should You See a Doctor?
Medical evaluation is recommended if rectal bleeding:
Occurs repeatedly
Is associated with abdominal pain
Appears mixed within stool
Is accompanied by fatigue or weakness
Occurs with unexplained weight loss
Happens in individuals over age 45
Even when bleeding appears minor, a physician should evaluate persistent symptoms to determine the underlying cause.
How Rectal Bleeding Is Evaluated
Evaluation often begins with a medical history and physical examination. Depending on symptoms, additional tests may include:
Anoscopy
Flexible sigmoidoscopy
Colonoscopy
Imaging studies
A colonoscopy allows direct visualization of the colon and rectum and is commonly used to identify the source of bleeding.
Related Conditions
Rectal bleeding may be associated with several anorectal and colorectal disorders, including:
Treatment depends on the underlying cause. Some cases resolve with dietary changes and medical management, while others may require procedural or surgical intervention.
Immediate medical attention should be sought if rectal bleeding occurs with:
Large amounts of blood
Dizziness or fainting
Severe abdominal pain
Rapid heart rate
Black or tar-colored stool
These symptoms may indicate significant gastrointestinal bleeding requiring urgent treatment.

Dr. Akhtar was professional, compassionate, and took the time to thoroughly explain everything before my procedure, which immediately put me at ease. The procedure itself went smoothly, and I truly felt I was in excellent hands the entire time. The office staff was also kind, organized, and attentive throughout the process. Iβm incredibly grateful for the level of care, patience, and professionalism I received. I would absolutely recommend Dr. Akhtar to anyone looking for a skilled doctor who truly cares about their patients and their comfort.β

I was crying in her office when she met me the first time. That didn't deter her. Not only that but she located the issue on my first visit. She is kind, caring, thorough and incredibly knowledgeable. She listens, spends time with you and doesn't talk over you. I've needed extra time to discuss my case many times and she has always taken the time that I needed. I've brought to her research I've read, and she always already knows about the publications. The staff are kind and helpful. I dont know what I would have done without finding her. Another health care provider recommended her highly as do I.β

Dr. Akhtar is compassionate, gentle, warm, and caring. I came to her in Glendale after having a horrific experience with a general surgeon in Flagstaff (where I live). The Flagstaff surgeon caused a hemorrhage that almost cost me my life, and then I developed stenosis after the emergency room surgeon βfixedβ the hemorrhage. Dr. Akhtar was kind enough to take me as a patient (most surgeons will not take a patient who has already been operated by another surgeon due to liability). She was very respectful and gentle in the examination, and was even able to fit me in for an outpatient procedure under sedation the very same day (knowing that I drove over 2 hours to see her). I could not be happier with her. She took the time to personally call me the next day to check on my progress. You do not see this type of doctor very often anymore. She is someone who is in it for the patient, and you can tell. I will continue to drive over 4 hours there and back to receive her high quality care and expertise. Cannot recommend her highly enough.β

I have not met many people who have as comforting a bedside manner as she does. I was educated properly every step of the way and felt confident doing so because of her caring demeanor. Her PA Moses is also so friendly and upbeat, helping to ease any tension you may be feeling prior to the exam. The energy both Dr. Akhtar and Moses bring was paramount to how excellent my experience was here. Given their specialty, it would be easy to have an uncomfortable experience. However, they go the extra mile to make sure you are comfortable and reassured before anything is done. I cannot recommend this practice enough. Thank you Dr. Akhtar for the genuine concern and care you provided to me! Both my body and my outlook on my health outcomes are better off because of you.β
